Steps to Success

A team member meets with the youth and their parent/guardian to discuss their challenges.

A private needs assessment is conducted with the youth and with the parent/guardian to obtain information on the youth’s behavioral trends and to help resolve issues.

Based on the assessment, our staff creates an individualized intervention plan and a team member is assigned to support in the plan.

Youth & Family Connections is a non-profit, community-based organization which focuses on prevention and intervention strategies, comprehensive assessments, case management and coordinated access to services.

We help an average of 500 youth under the age of 18 each year who are struggling with life issues to offer positive alternatives for growth and support no matter what stage of life they are going through.
